What is chronic care management (CCM)?

CCM is a Medicare program that helps adults with chronic conditions manage their health and prevent serious problems. Some of the chronic conditions that CCM helps with are:

Who is eligible for chronic care management? 

To qualify for CCM, you must be a member of Medicare. Enrollment starts at age 65, and you must have two or more chronic conditions to participate in the CCM program. 

Chronic conditions are those expected to last anywhere from a year to the rest of your life.

What does chronic care management include?

CCM

Chronic care management gives you a dedicated chronic care manager, a health care specialist who follows up with you once a month. These appointments are through telehealth. 

This CCM check-in involves reviewing your medication, answering your questions, and helping you schedule any necessary medical care. 

Remote patient monitoring (RPM) device

Most people in the CCM program qualify for a remote patient monitoring device, another Medicare program that works hand-in-hand with CCM. RPM devices click to the side of your bed. 

They electronically monitor your heart rate and blood pressure and transmit this information to Pulse Primary Care. This allows them to act quickly if you need help. 

Remote therapeutic monitoring (RTM)

RTM is ideal for people who take a lot of medications. Watching out for interactions and side effects can get confusing, so Pulse Primary Care takes that burden off your shoulders with RTM. They send you an RTM device for free. 

All your medications go inside the smart device, which tracks when you take them. If you accidentally miss doses for a few days or take medication too often, your smart device alerts the practice so they can help you get back on track.
